For immediate access to your records, you may log in to your patient portal and this will allow you to view and download/print your records. To access the portal, please utilize the link provided below.
If you prefer not to use the patient portal, an alternative option is to complete the medical records release form listed below. If your intention is to have your records sent to another healthcare provider, kindly provide us with the name of the provider, their phone number, and fax number and records will be faxed.
